The survey is available in English and Spanish and will take between 5 to 10 minutes to complete. You will need the 16 digit survey code number and Internet access. At the end of the survey you will have the option to enter a sweepstakes gift card drawing as a reward for your time and feedback. All feedback information provided will be kept private.
