The Rowe v. UniCare Lawsuit Settlement is in the jurisdiction of the United States District Court; Northern District of Illinois, Eastern Division and is case number 09-CV-02286. The lawsuit claims that UniCare violated insurance laws and did not adequately protect the Private Information when private information such as Class Members’ names, Social Security numbers, income information, employment information, phone number, medical history, prescription records, medical diagnoses, medical treatment history were available over the Internet to unauthorized users. This incident occured in the years 2007 and 2008. UniCare denies all wrong doing in the case but has agreed to settle.
If you recieved a privacy violation letter from UniCare between between April 1, 2007 and June 30, 2008 you are considered a class member of the UniCare Life & Health Insurance Claims Class Action Lawsuit. All class members must file a claim, more information on how to file a claim can be located at www.unicaredataclaims.com. Claim forms must be postmarked by November 19, 2011, or 120 days after you receive your notice whichever is the later. If you are a class member and do nothing you will not get any type of settlement. The claim form is available online at the class admin portal and Adobe Reader will be needed to view and print the claim form.
